For your first office visit, please print out, complete, and bring in the hard copies of the 1. Health History2. Consent to Treat, and HIPAA forms. If you are seeking fertility support, please also fill out the 3. Fertility Intake Form as well. (There is a $5 charge to fill out forms in the office.) I also recommend bringing any lab or test results or BBT charts as they may be able to help me determine the best course of treatment for you. Thank you! I look forward to our first visit. 

What To Expect

Your first visit will take approximately one hour and 15 minutes; follow-up visits are approximately 50 minutes. I will review your complete health history. Some of the questions may be unusual, such as detailed questions about your digestion, even if you have come to see me for fertility or pain. Oriental Medicine treats the body as a whole and that means I will consider all of the body’s processes, both emotional and physical. I will also take a look at your tongue and take pulses on both hands. All of this information will paint a picture of your condition and your constitution and help me to formulate the best personalized diagnosis and treatment plan for you.

 Maya Abdominal Therapy first-office visits run approximately 80 minutes and follow-up visits are approximately 50 minutes.

 You will be clothed or draped for your acupuncture treatment. Please wear loose filling items that can easily be rolled up over your knees and elbows or I can provide you a loose-fitting clinic outfit or drape. Please have a little something to eat before your treatment.

 Most people find acupuncture very relaxing. Needling sensation depends on individual sensitivities.

 For your safety, I use only sterile, single use, and disposable needles.

Please wear a face mask. If you do not have one, one will be provided for a $5 charge.

 Parking

Free unlimited street parking is available on Telegraph Avenue in front of my office or in the surrounding neighborhood streets. I recommend checking out 63rd Street first as it is a little less crowded than North Street. Please read parking signage as street cleaning parking may apply on some Mondays and Tuesdays.

 Public Transit

I am located within a 15 minute walk from both the Ashby and Rockridge BART stations and am located on the 6 (Telegraph Ave.) transit bus line and 2 blocks from the 13 (Shattuck Ave.) transit bus line.
